Diabetes, especially Type 2, is the most prevalent disease in modern society. Millions of people worldwide are afflicted, costing health care systems billions of dollars with an increasing trend every year. It brings suffering and frustration to their daily life.

To counter the disease, new drugs, and thousands of published articles are hitting the market in an attempt to educate the people on how to reduce their sugar intake. Even so, people continue to battle against this disease with continued frustration.

Diabetes or hyperglycemia develops when sugar metabolism is inefficient and causes an increase of blood sugar above the normal range. Efficient sugar metabolism, its utilization and balancing the required blood sugar level depends on many factors, starting from: digestive pathways, pancreatic functions, liver functions, kidney functions, muscle functions, thyroid functions, brain functions, and how all the body cells are utilizing glucose for energy production effectively.

The body has a unique mechanism to balance sugar level in the circulation. When intake of carbohydrates as a food undergoes digestion process, it enters as glucose (sugar) into the liver and passes into circulation. As soon the circulatory sugar is rises the brain instantly signals the pancreas to release insulin and signals the liver to control dispersing sugar into circulation by utilizing storage mechanism. Similarly when circulatory sugar drops less than the normal range, brain signals the pancreas to release glucagon and signals the liver to release sugar from storage. This is one of the principal feedback systems body follows for glucose (sugar) homeostasis. In order to maintain such a feedback system, your body cells, including brain cells, pancreatic cells and liver cells must function properly.

In order to function properly cells must produce energy from sugar efficiently and transfer the energy to cell membranes/tissues and to organs for their specific functions. Collectively when all the systems and organs function properly that is considered “LIFE”.

Inefficient sugar metabolism, leads to ineffective sugar utilization by the body cell and thus inefficient energy production, which cause body cells to be more stressed and become dysfunctional as time passes. For example, if it is a pancreatic beta cell that becomes stressed, the insulin secretion can be impaired. If it is liver cell that becomes stressed, a series of metabolic pathways can be impaired. Inefficient energy production of pancreas and liver can also impair signaling processes of intra-cellular as well as extra-cellular actions. For example, in hyperglycemia (high sugar) where blood sugar is higher than normal, the liver still produces more sugar from fatty acids, lactate and amino-acid. This is due to the impairment of signaling communication between brain cells, pancreatic cells and liver cells.

It must be emphasized how cells from various organs, muscles, lymph and other body cells become stressed and develop insulin resistance. There are instances, where pancreas secretes insulin but that is not utilized effectively.

The current diabetes medications including insulin therapy deals with controlling sugar levels for the time being, not necessarily correcting the dysfunctional body cells including pancreatic beta cells. Basically, there are four groups of medications besides insulin. One group of drugs is used to blocks K-ATP in the pancreatic beta cell membrane, creating a voltage gradient opening for Ca2 influx to aid to release insulin. Normally cell mitochondria produce energy ATP and is used to blocks K-ATP in beta cell membrane. Now, by bypassing cell’s normal pathways the drugs are making the cells more dysfunctional as time passes, thus more and more medications are required as days go by. Another group of drugs blocks sugar synthesis from fatty acids, lactate and amino acids by the liver. Normally the liver produces sugar from fatty acids, lactate and amino-acids in fasting condition when blood sugar is low. Once again more and more medications are needed as time passes as it is not correcting the cell’s dysfunctional coordination. The 3rd group of drugs, where it excretes excess sugar through the kidney and may cause less sugar utilization by the body cells making some other cell dysfunctional. The 4th group of drugs control sugar (glucose) absorption from GI system blocking an enzyme pathway and may cause less sugar utilization by the body.

The body needs food as carbohydrates, fats, and proteins for energy, cellular integrity and its protectionism. In order to utilize the foods it undergoes thousands of bio-chemical processes every minute during life cycle. The body also produces thousands of hormones and enzymes, and during synthesis and degradation it also needs vitamins, minerals, micro-nutrients and amino-acids in the bio-chemical reactions. Each cell of each organs system functions differently using different bio-chemical processes and collectively the functions outcome is considered ‘LIFE’. However, some organs use only sugar for their energy, such as brain cells and blood cells.

Glucose metabolism can follow many pathways providing not only energy supply but also many other important metabolites for cell growth and function. It may be transported into the cell through glucose transporter GLUT 1, GLUT 2 and GLUT 4. Normally GLUT 1 and GLUT 2 mediates insulin-independent and GLUT 4 mediates insulin-sensitive glucose transport. Once glucose is inside the cell it must phosphorylated to glucose-6-phosphate (G-6-P) by an enzyme called hexokinase. The enzyme has a cofactor of Mg (magnesium). More glucose converts to G-6-P while more glucose is picked up from the circulation thus reducing the glucose from the circulation.

The next step is utilization G-6-P into energy production pathway and pentose phosphate pathway (PPP, which is divided into oxidative pathways and non-oxidative pathways). The primary function of the oxidative PPP is to form NADPH (Nicotinamide Adenine Dinucleotide Phosphate), which is important in combating cell stress free from reactive oxygen species (ROS). B-Vitamins also play an important role in the NADPH. In addition, liver cell may utilize G-6-P to store glucose as glycogen. That means efficient liver cell plays important role in glucose homeostasis.

More glucose utilization by cells/organs means more reduction of sugar in circulation.
